Teller Report says the Hyundai minivan crashed into a truck moving in the opposite direction.  Five people died on the spot, including a six-year-old child.  Three more victims were hospitalized, where one of them died.  According to the preliminary version, the cause of the accident was the departure of the minivan into the oncoming lane due to the actions of its driver, since the road conditions at the time of the accident were normal, Teller Report notes.
